This is chronic rheumatic valvulitis involving the mitral valve. The right panel illustrates the marked shortening and thickening of the chordae tendineae. In the left panel the enlarged left atrium is opened to reveal the "fishmouth" shape of the mitral valve, which is functionally both stenotic and partially insufficient. Mural thrombi may occur in such an enlarged atrium, and one is seen at the right.
