Case 59

From the appearance of the tissue seen here with Congo red stain, what abnormal serum laboratory test finding would likely be present in this obese, middle-aged patient?

This is an islet of Langerhans in the pancreas. The Congo red stain reveals amyloid deposition, a finding that can be seen with type II diabetes mellitus. This patient will have hyperglycemia and an increased hemoglobin A1C. The protein amylin is secreted along with insulin, but builds up in the islets of some (but not all) type II diabetics. Whether this deposition is the cause or consequence of abnormal beta-cell response to hyperglycemia is not known. Over 80% of persons who develop diabetes mellitus are overweight. For the first time in human history, at the beginning of the 21st century, there are more overweight persons than malnourished persons on earth. Thus diabetes mellitus will become even more common in the future. The solution: share, and everyone is better off.
