What event documented by the CT scan resulted in the gross appearance seen here months later?

The CT scan reveals a large right hemispheric cerebral infarction. From this view it appears to stem from occlusion of the internal carotid, though multiple views revealed involvement mostly in the territory of the middle cerebral artery. The section of midbrain with substantia nigra reveals atrophy of one cerebral peduncle as a result of loss of upper motor neurons and Wallerian degeneration of descending corticospinal tracts. The patient had a left hemiparesis as a consequence.
