Monozygous twinning may be accompanied by a vascular anastomosis across the placenta from the circulation of one fetus to the other. Such a twin-twin transfusion syndrome can put either the "donor" twin (from anemia) or the "recipient" twin (from too much blood flow) at risk for demise.
