One should do an excision to remove the lesion. If it is possible to encompass the entire lesion by either method, it is best to submit the whole specimen to avoid sampling error. If the whole specimen cannot be easily removed, the area that is most elevated should be biopsied, or if the entire lesion is flat, the darkest area should be biopsied.
