Biopsy: H&E:  Clinical erythema--a nonspecific inflammatory
infiltrate in the dermis, often arranged around dilated capillaries.
Papules and pustules--nonspecific collections of lymphocytes and histiocytes in the dermis, the pustules often have PMNs in or around the follicles. However, MOST of the infiltrate is perivascular. In about 10% of cases there are foci of a granulomatous infiltrate, i.e., there are islands of epitheloid cells surrounded by a round cell infiltrate and occasional giant cells.
IF:  IgG and/or IgM and/or complement are often found at the DE junction and/or in the dermal collagen on direct IF. Indirect IF shows occasional IgM ANA.
