Bacterial Skin Infections
- Most infections are caused by staph and/or strep.
- They can usually be treated with dicloxacillin 250mg 4X a day for 10 days, or the same dose of erythromycin.
- An alternative treatment is rifampin 300 mg 2X/day for 10 days.
Fungal Infections
- An effective OTC antifungal/yeast cream is MicaTin, usually applied 2X/day for 2 weeks.
- Most diaper rashes over 3 days duration have 2% yeast infection.
- For fungal infections involving the hair, one must use griseofulvin 10-20mg/kg/day until clear (about 2-3 + months).
- To clear fungal infections of the nails it takes about 8 months to regrow fingernails and about 18 months to grow new toenails.
Herpes
- Herpes simplex is treated with acyclovir 200mg 5X/day for 5 days, or 200mg 3X/day continuously for prophylaxis.
- Herpes Zoster is treated with 800 mg 5X/day for 10+ days.
Warts and Molluscum
- Painless
- Occlusal HP daily till clear
- Canthrone Q 2 weeks till clear
- Efudex 5% (5-FU) 2X/day
- Painful
- Freezing for 30+ seconds
- Electrodesiccation & cutterage
- Laser
- Bleomycin injections
Common Childhood Infection
- Each common childhood infection is quite well defined, and the seven are identified herein.
General Guidelines
- Fungal infection of the nails can be treated with Naftin Gel 2X/day for 6+ months, or griseofulvin 500-1000 mg.day.
- Kerions are treated with triamcinolone 10mg/cc intralesionally, and griseofulvin 10-20mg/kg/day till clear. Injections can be repeated every 4 weeks.
- For most bacterial infections, a gram a day keeps the doctor away.
- Fluctuant (fluids/pus filled) lesions such as furuncles MUST be incised and cutterage performed to break open loculations of fluid and remove necrotic material.
