If an ulcer has any necrotic tissue, it then has to be removed. The necrotic tissue simply provides potential nutrition for the bacteria, and is of no benefit to the healing process. I found that ulcers on the malleoli are very sensitive and the slightest touching or movement induces significant pain. I would recommend injecting anesthetic before debriding these.
