Using Handheld Computers for Admission Order Entry

ABSTRACT:
Introduction: Computerized physician order entry (CPOE) systems allow physicians to directly input their orders into the hospital computer system without the use of a clerk to interpret the meaning of the physician's written words. CPOE systems can crosscheck for medication dosing errors, allergies, and formulary-based substitutions. Unfortunately, most hospital-wide CPOE systems carry a multi-million dollar price tag that few institutions can afford at this time. This project demonstrates that handheld computers may be a viable and less expensive alternative that can still enhance efficiency and decrease errors.
Description: The Syware Visual CE program was used to create an order entry program for the Jornada Handheld PC running the Windows CE operating system. The program allows hospital approved order templates to be utilized with a series of drop-down lists and check boxes. The program will alert the user of basic allergic reactions and medication errors, and will suggest alternatives to non-formulary medications. Once the orders have been entered, they are printed and placed in the chart. As no interface yet exists with the hospital system, a clerk will still need to log the orders.
Conclusion: This system may improve the efficiency and accuracy of admission order entry, and will likely increase the use of hospital approved order templates. With a cost in the thousands rather than millions, it may be a viable alternative for smaller hospitals for which full CPOE systems are still years away from budgetary realization.

Handheld computer devices are increasing in popularity, but their full potential has yet to be realized within the medical field. SOL participants are looking for real-world applications for this technology and will be interested in exploring this 'home-grown' program.
