    For Online Conference Registration using a credit card, please provide us with the information below. The areas marked by an asterisk "*" MUST be completed. All prices listed are in U.S. Dollars. A charge to "GOLD STANDARD MULTIMEDIA" will appear on your credit card statement. Cancellations accepted through July 15, 2001; a $50 administrative fee will be retained for cancellations.  A bank transfer fee of $15 may be charged for foreign currencies; therefore, credit card payment is preferred.

    If you have a complimentary registration, you must register by printing this form and mailing or faxing it to Leah Smith at Gold Standard Multimedia (see contact information below).  Include payment for the Welcome Dinner and/or the Grand Extravaganza if you are attending either event.  All attendees must register.

Registration for preworkshops by people NOT attending the conference will be open after July 10, 2001. At any time, you may register to attend preworkshops only. However, we will hold your registration form and payment until July 10, when we will begin assigning the remaining space in the preworkshops on a first-come, first-served basis. All attendees MUST be pre-registered and paid through Gold Standard Multimedia in Tampa, Florida, USA.

    Call for Submissions for the Slice of Life Annual $1,000 Student Software Award

    Deadline is July 23, 2001. A letter from the faculty person involved with the project, a copy of the software and instructions on how to use it should be submitted to Suzanne Stensaas. The submission does not need to use the Slice of Life or Slice of Brain videodiscs and may run on any platform and be in any language.  Mail to: Suzanne S.
